This service supports Members’ choice to live in and access opportunities in their communities through services offered in Supported Living. Members using Supported Living services receive a variety of interventions designed to maximize their independence, including habilitative therapies, skill development, behavior intervention, and sensorimotor development.
Individually designed living arrangements allow a member to live in their own homes or apartments. Support staff provide a variety of services and interventions designed to maximize the functioning of members. Generally, one (1) or more members reside together in a private residence that is leased or owned by the member(s) and/or the member(s) representative. The focus of this service is to provide habilitative support to these members based on the collective need for direct staff support to eligible members who have chosen to reside together and share their resources. Members living on their own are expected to pay for their own expenses, such as their rent, utilities, and groceries. Through guidance and supervision, the family and care providers allow the person to smoothly transition to society. This service includes assistance with cleaning, social skills, shopping, and maintaining a budget. This service provides more flexible options for Members who wish to take a more active role in managing their living environment, while receiving the support that they need to thrive in that environment. Members live as independently as possible and develop or enhance their independence in their home, participation in their community, and relationships with others.
